White, five-petaled flowers 25 mm in diameter. Petals are conspicuously veined. Basal leaves are cordate with long petioles, I measured the blade at 15 mm wide and 20 mm long. Flowering stems have a single leaf, clasping the stem about mid way.

Flowering stem is square. Plants are glabrous.

Native to moist, shady places in Saskatchewan's parklands and boreal forest.

Budd's Flora lists the height to 30 cm, the plant in the above photo was 32 cm tall.
